That is absolutely insane and ridiculous. I don't know much about the medical profession, but taking the form from you after you specifically asked that it be destroyed? Um, yeah. That hits me ENTIRELY wrong, as it sort of voids the whole thing you sign. And even witholding information in the first place! Otherwise you wouldn't have signed. Arr at them.
I've never been to a psychiatrist or anything, but there have to be some good ones out there. I guess it's sort of like you have to dig through the dirt to get to the gold. Or whatever. I'm just talking out of my ass.
